Welcome to Piedmont Equine
Thank you for choosing Piedmont Equine to serve you and your horse! We have developed a step-by-step guide to help you navigate the first appointment, either at our facility or on your farm. If email and on-line forms are not comfortable for you, please allow our office staff to assist you prior to your scheduled appointment.

For emergencies, please call our office number to be connected to a veterinarian.
Doctor to client relationships are important to us, and we strive to provide you with the best match possible. Please share with us your patient’s history, your discipline of choice and/or the planned use of the horse.
Additional Forms
If you are sending your horse by a trainer to receive services or planning to leave your horse at the hospital, please complete the “Client Consent Form”. This will reduce hospital check-in upon arrival.
If your horse has been seen by another veterinarian, those records would be helpful to our team in continuing diagnostics and treatments. Please complete the “Patient Referral” form.
Upon arrival at Piedmont Equine Associates:
Please enter the first driveway marked by a large Piedmont Equine sign. As you approach the gate, it will automatically open. Please stay to the left against the black board fence when parking. This will allow multiple trailers to pass through the parking area during your visit. Please leave your horses on the trailer and check in at the main office. The main office entrance is located on the large porch with a chandelier. Our office staff will be available at check-in to assist with any other questions that you may have.
